The Tammy D. Martin Trips, Dreams, and Memories Foundation is accepting nominations for our first family. We hope to send the family on vacation by the end of summer 2009! The actual date will be dependent on the amount of donations we receive.

How much does a trip cost?
Trips will vary based on the destination, number of travelers and changes in the travel market.

The Foundation has established a per person "guideline" that includes airfare, car rental fees, accommodations, tickets/admission (if required), meals and incidentals (souvenirs, entertainment, etc.) The Foundation will only cover the actual expenses for the trip.

Who is eligible?
The Tammy D. Martin Trips, Dreams, and Memories Foundation was established to help parents who are facing a life-threatening medical condition take a family vacation. The purpose of the vacation is to provide an opportunity for the family to enjoy time together outside of treatments, medical appointments, and the day-to-day hardships of dealing with a serious illness.

It is important that the person who is ill be able to actively participate in the vacation. If selected for the program, members of the Foundation will work closely with the family to plan a vacation that is appropriate for their medical situation.

How do I nominate a family?
The Tammy D. Martin Trips, Dreams, and Memories Foundation will post information on our website when we are accepting nominations.

Anyone can nominate a family, including the family themselves. To nominate a family, please review our Eligibility Criteria. If the family meets our eligibility criteria, you can submit a Nomination Form.
