What we provide
The Tammy D. Martin Trips, Dreams, and Memories Foundation was established to help parents who are facing a life-threatening medical condition take a family vacation. The purpose of the vacation is to provide an opportunity for the family to enjoy time together outside of treatments, medical appointments, and the day-to-day hardships of dealing with a serious illness.


Criteria
The following criteria will serve the selection process by allowing fair, unbiased and compassionate selections, and provide stewardship of funding to the greater good of all. A scoring system will be used to ensure all applicants are evaluated in a fair manner.

  • One parent in the candidate family must have a life-threatening medical condition. Life-threatening is defined as a disease, chronic or acute, in which the patient is not expected to recover or be able to sustain life more than one year. Must be attested by two treating physicians at the time of application.
  • It is important that the parent facing a life-threatening illness be able to actively participate in the vacation. A nomination can be made without the family's knowledge, however, if a family is being considered, the family must be informed and the parent facing a life-threatening illness must complete the required application process.
  • The candidate family must have at least one child between the ages of 4-18 years of age at the time of travel.
  • Travel is limited to immediate family only. Immediate family is defined as:
    • the parent facing a life-threatening illness
    • a spouse or partner who is living in the home
    • all biological children, step-children, and adopted children under the age of 30
  • The parent facing a life-threatening illness must be healthy enough to travel. Must be attested by all treating physicians at the time of application, at regular intervals while in the program, and at the time of travel.
  • The parent facing a life-threatening illness must be a legal resident of the State of Colorado for at least 12 consecutive months at the time of nomination. An exception may be made if the parent is temporarily residing out-of-state for medical treatment.
  • The candidate family must be unable to financially afford a vacation with their own resources. This does not mean destitute. Quality health care in this country is expensive to say the least, and even the best insurance plans are not covering the full cost of being seriously ill. Most patients cannot continue to work during treatment at the same time that medical bills begin to accumulate. Even for families who live well within their means, spending money on a vacation at a time like this could be impossible.

How do I nominate a family?
The Tammy D. Martin Trips, Dreams, and Memories Foundation will post information on our website when we are accepting nominations.

Anyone can nominate a family, including the family themselves. To nominate a family, please review our Eligibility Criteria above. If the family meets our eligibility criteria, you can submit a Nomination Form.
